Telemedicine Informed Consent

Effective Date: 07/01/2025

At ActuateCare, we provide health care services through secure telemedicine technology. This allows you to connect with licensed physicians and other qualified health professionals from wherever you are. By using our telemedicine services, you acknowledge and understand the following:

What Telemedicine Means

  • Telemedicine uses secure, real-time video and/or audio technology, along with the electronic exchange of medical information (such as images, test results, or health data).

  • Your provider may be located at a different site than you, but the quality and confidentiality of your care remain the same as an in-person visit.

Important Things to Know

  • In-Person Care May Be Required: Some conditions may not be appropriate for telemedicine and may require an in-person evaluation or treatment.

  • Privacy & Security: Your telemedicine visits are protected under HIPAA and other applicable laws, just like any other health care service.

  • Medical Records: You have the right to access your records related to telemedicine visits in accordance with state and federal regulations.

  • Emergency Situations: Telemedicine is not for medical emergencies. If you experience a life-threatening emergency, call 911 immediately.

  • Information Sharing: Your health information may be shared with other providers or health care entities for treatment, billing, or operational purposes, as permitted by law.

  • Scope of Care: The provider will determine whether telemedicine is suitable for your condition. There is no guarantee of diagnosis, treatment, or prescription.

Acknowledgment

By choosing to use ActuateCare telemedicine services, you acknowledge that you have read and understood the information above. You also acknowledge that ActuateCare’s Notice of Privacy Practices has been made available to you.
